It's heavily romanticized. In general, depending on which character the person is mentally portraying themselves as, there's two different instances.
On one side, it's a saviour complex. Save the heroin, they will have immense graditude and fall in love with you (and then everybody claps).
On the other side, it's a rescue fantasy. A perfect victim, a damsel in distress that wishes somebody (insert hot love interest here) will swoop in to solve their problems for them. Somebody they can completely rely on without having to do too much themselves. A protector.
.. There's honestly more to it and this is just scratching the surface (not including the smut). For the savior side it's mostly seen in male centered media, of the MC saving a beautiful woman from a dangerous situation (alt. things like SA), she'll be greatful, realize he's "not like other men" but strong and reliable and eventually become infatuated and loyal to only him. Basically the typical male insert seen everywhere in shounen animes and even western media.
For the rescue side it's typically the same thing except it's just reversed roles with the MC being in the helpless and afraid position. Needing someone to rely on. The self insert seen in often in shoujo related materials (or media catered twords women) I feel like I could put much more in this section but honestly you probably get the gist of it.

Smyrna & Capri is one of my favs omegaverse. It's not a perfect one since you could say the beginning is dubcon, and the characters are reluctant or even regretful of their relationship at times. However, I think the way their relationship grows over time is great. It's technically Alpha x Alpha (although it does have mpreg), and the only Omega character I remember is kind of irritating to the plot tho I don't think it's due to their secondary gender but more of the personality of the character.
Maybe if omegaverse stories were purely smut and didn't try to expand on the societal views then I would see it differently. It's just that there are a lot of parallels to how women are treated in real life along with fighting for rights or equal treatment in comparison to Omega characters (male or female) that just makes it poor taste to me most of the time. When the Omega who was fighting for their rights gets assaulted or mated, suddenly forgets about wanting their independence/better treatment for their secondary gender, and plays a docile live at home partner after have multiple kids which is then showed to be like their only purpose and cycles back to the narrative that they were originally fighting against (that Omegas are only good to be bred). It feels like subtle messaging... (yes, I know breeding kink is a thing)
