It's not exactly the vibe you're going for but 'Kill the Villainess' ML is very much an NPC type in the best way and ALSO a loyal knight type and their relationship breaks my heart - lots of angst and she's quite cruel (not as much as Thalia but she very much stays a villainess till the end)

I think it's the other way around. MC is the one misunderstanding her own situation (rofan), but the others' perception of her is more accurate (cosmic horror).

She might feels a bit caring for some people especially children, but not to the point of thinking they are real.
