I don't know if that image is from the present time or a flashback. Some panels seems to be kind of symbolic, not fact... But, good point!. I didn't think much about it. It's all a bit messy, for me at least.
He did took her ultrasound at his home, remember when he pick up something in her trash where they were hs Mincheol and her picture. Also it was stated in another chapter that he wanted to use it as his card , but end up not doing because he don't want to pull that kind of love without her consent. Maybe his plan was to made her enters on her family with a fake pregnancy idk
This is a flashback, in earlier chapters it was already shown Na Hae Soo had a miscarriage, which is partly the reason why Minsh*t started acting like a d*ck towards her, blaming her for losing the baby (also implied).
