I can assure you, I am the only one who got rage baited by dis manhwa..I have dropped it before like three four years ago, hoping my taste would change. Picked it up and finally read it recently...there was nothing exciting about it.

Yes STRANGERS AND PARENTS can find the child beautiful but they never say that child is my first love. Yuri is associating his first love with his first love not from a place of parental love

I think there is a translation error because from my pov while reading knowing the concept of different languages and how korean operates I think yuri focused more on the “made you ache” part
And so he recalled the time he had to wait for xinlu to pass out so he can help him
He obviously thought that xinlu was an adorable kid and so seeing him struggle may have made his heart ache like how you would when a baby you love like your niece is getting hurt because of a needle so she cries or upset and you’re like “poor thing :(“ kind of vibes or maybe the semantic can be understood differently
I have to emphasize that i might be wrong but this is what i understand from the chapter so maybe another perspective might help you understand why there is a lot of people trying to explain that futher more maybe there is context in the novel or something too

Gosh I am too lazy to be typing all of dis.. just read my original comment. I said Yuri is in love with ADULT XINLU, not the child. He later on goes to say that it is necessarily not "love" he felt but was mere "admiration". And let me remind, Yuri himself doesn't know the concept of love CLEARLY. That's the flaw of his character.

Grow some Comprehension skills, Yuri have NEVER loved before and now he is in love with ADULT Xinlu, thinking about the first time they met. He did not say he was in love with Xinlu SINCE they met, or SINCE he was young. It's just that the person he love NOW happens to be the person ONCE he saved and found captivating. I am too lazy to be explaining the same thing I already did in my original comment..

What exactly is the "first love" in this context? To my understanding it's nothing sexual. It's something more along the lines of how artists love their muses. It's something more parallel to loving God. You don't have sexual feelings for them. You admire the they posses. You admire them. I hate to say it people have no reading comprehension nowadays.
