Lowk, I want the ML to eventually find out about Evelyn's illness and accept it for what it is. Once they've both come to terms with it, they finally confess their love for each other, and Evelyn decides to spend whatever time she has left with her husband.

And just as the ML wakes up and realizes that she's gone, the doctors come rushing in with the cure.

Then, one day, while wandering through his manor in a drunken daze, he comes across a portrait of Evelyn. Something about seeing her face finally makes the reality sink in.
And somehow, that becomes the moment he begins pulling himself back together.
Inspired by Evelyn's portrait, the duke returns to a childhood hobby he had long abandoned, drawing. He starts practicing again, slowly getting better and better, and whenever he has the time, he draws Evelyn.
Years pass.
With age his mind and sight slowly fade away, and his drawings of Evelyn become more distorted as time passes.
Then, one day, as he sits in front of the portrait that first pulled him out of his despair and he slowly drifts into a deep sleep.
In his dream, Evelyn is standing in a field of flowers on the other side of a bridge, looking as beautiful as the day they got married.
He approaches her slowly, almost afraid that she'll disappear if he moves too quickly.
Then he starts running.
As he crosses the bridge, time seems to reverse. His body grows younger. His mind becomes clearer. He feels alive again.
And by the time he reaches Evelyn, he looks exactly as he did on the day they were married.
He takes her hand.
Together, they walk away into the forest of the afterlife, leaving the mortal world behind.
The next morning, at dawn, a servant discovers the duke sitting peacefully in front of Evelyn's portrait.
He is no longer breathing.
But on his face is the most content smile a man could ever have.
