In most omegaverse stories the male omega give birth through c-sections. They don't get periods either, and there is no mention of them having ovaries. Their heats are the only time they can get pregnant, so maybe it's like only during that time does the egg gets released from the supposed ovary since it's not an active organ. But if a miscarriage is shown in an omegaverse, they usually bleed through their butt, and to remove the miscarried baby they would have to go through a c-section.
