It's a horror manga honestly, it was a messed up story but i kinda wanna see it to the end. A guy was transferred to an orphanage (if i'm not mistaken) where he would be a new teacher (??) But then he found out the kids there were not normal because they do gory work. This orphanage was actually a human disposal place and the one doing the killing were the kids. Some defect yakuzas, criminals, etc were sent here to get killed. Then there was a plot twist where the guy transferred here was actually a former member of this so called orphanage.

MC cheats on ML with a girl, and starts a serious relationship with her for reasons even he doesn't know why. He flip flops around ML and himself like "yeah I'm normal and have a girlfriend, so what, im straight and i hate you" to "I love only you baby, stay with me forever" to "I only love my girlfriend" to "why do I have a girlfriend" "why do I have a boyfriend". Like at one point he's planning possibly starting a family with his girlfriend even though hes literally about to die. This is because the tumor is messing with his brain, and pressing against the part in charge of making decisions and is messing with his personality.
Everyone finds out that MC has brain cancer after he collapses while puking up blood in front of someone, and then it finally clicks to them why he's been doing and saying stuff he normally wouldn't, cause his brain is literally dying so it's messed up, and he should have had a care taker to watch over him for a while now.
After that it gets to the point that MC is just permanently hospitalized and his condition gets worse.
Also MC breaks up with the girlfriend, he didn't actually like her in that way, but he thought he did because of the effects of the tumor scrambling his mind. He is now permanently in a relationship with the ML.
The ML visits him daily in the hospital to take care of him, but then MC's mom has to constantly argue with the hospital staff because the other patients in the room are homophobic, and complain about them being in the same room as them. The treatment isn't really stopping the pain or vomiting at this point, and he's drastically losing weight, MC is being harassed daily by the other patients, his sister also harasses him and tried to beat him up multiple times to the point her girlfriend had to pull her off of him, and the long hospital stay is draining his family's money. So MC tells ML he doesn't want to spend his last days in this hospital being yelled at, and that all these treatments the mom is having the hospital do for him to prolong his life are all painful and just feel like torture. So they both decide to help him escape the hospital, and rent a vacation rental near the beach without telling anyone where they went.
They spend time in the beach town together, just living there, going out, and trying to make his last days calm. Same as in the hospital, his health is quickly deteriorating. He vomits daily, coughs up blood multiple times a day, it can be hard to walk at times, his mind if even more of a mess, and now it's so painful he has suicidal thoughts. MC goes back and forth between telling ML to forget about him and leave him because move on once he's gone and not wallow in sorrow (especially since the ML has said flipantly multiple times that he doesnt intend to live in a world without the MC), to the next second where he is begging him not to leave him alone because he's scared and doesn't want to be forgotten when he dies.
Anyways, one day he is very sick, he's even waking up in bed coughing up blood, crying because he's in so much pain, and says multiple times he wants to die because he is suffering trying to prolong his life. The ML tries to reasure him that he will always be there for him, he won't leave him alone, and that he will feel better in the morning. Later that night after ML cleaned up the blood off of MC and the sheets, and they had gone back to sleep, ML wakes up and sees that the MC is gone.
He runs out in the snow frantically to try to find him, and he sees MC's slipper near a cliff on the beach, looks down, and it's implied that MC is dead down there. So the ML reiterated that he promised he would never leave him alone and always be with him. Then he jumps off the cliff. They now will always be together. The end.
