Transgender: is a term for those who's sex(external/biological) does not match their gender(internal/psychological).
They don't always get "Sex Confirmation Surgery".
Clothing has been pointlessly gendered for some reason even though it has no gender or is gender neutral. Heels used to be for men and so were skirts.

transgender is (like everyone else is saying) when you transition from one gender to another, and no you dont need to be dysphoric or have a surgery to identify as the gender you are most in touch, or comfortable with.
